This changes the awkward approach where architectures provide init functions to determine which levels they can provide large mappings for, to one where the arch is queried for each call. This allows odd configurations to be allowed (PUD but not PMD), and will make it easier to constant-fold dead code away if the arch inlines unsupported levels. This also adds a prot argument to the arch query. This is unused currently but could help with some architectures (some powerpc implementations can't map uncacheable memory with large pages for example). The name is changed from ioremap to vmap, as it will be used more generally in the next patch.
