In the case of SEV-SNP, whether or not a 2MB page can be mapped via a
2MB mapping in the guest's nested page table depends on whether or not
any subpages within the range have already been initialized as private
in the RMP table. The existing mixed-attribute tracking in KVM is
insufficient here, for instance:

  - gmem allocates 2MB page
  - guest issues PVALIDATE on 2MB page
  - guest later converts a subpage to shared
  - SNP host code issues PSMASH to split 2MB RMP mapping to 4K
  - KVM MMU splits NPT mapping to 4K
  - guest later converts that shared page back to private

At this point there are no mixed attributes, and KVM would normally
allow for 2MB NPT mappings again, but this is actually not allowed
because the RMP table mappings are 4K and cannot be promoted on the
hypervisor side, so the NPT mappings must still be limited to 4K to
match this.

Implement a kvm_x86_ops.gmem_validate_fault() hook for SEV that checks
for this condition and adjusts the mapping level accordingly.

This is a misleading name.  The primary purpose is not to validate the fault, the
primary purpose is to get the max mapping level.  The fact that this can fail
should not dictate the name.

I also think we should skip the call if the max level is already PG_LEVEL_4K.
Something _could_ race and invalidate the RMP, but that's _exactly_ why KVM
guards the page fault path with mmu_invalidate_seq.

Actually, is returning an error in this case even correct?  Me thinks no.  If
something invalidates the RMP between kvm_gmem_get_pfn() and getting the mapping
level, then KVM should retry, which mmu_invalidate_seq handles.  Returning
-EINVAL and killing the VM is wrong.

And IMO, "gmem" shouldn't be in the name, this is not a hook from guest_memfd,
it's a hook for mapping private memory.  And as someone called out somwhere else,
the "private" parameters is pointless.  And for that matter, so is the gfn.

And even _if_ we want to return an error, we could even overload the return code
to handle this, e.g. in the caller:

Side topic, the KVM_MEM_READONLY check is unnecessary, KVM doesn't allow RO memslots
to coincide with guest_memfd.  I missed that in commit e563592224e0 ("KVM: Make
KVM_MEM_GUEST_MEMFD mutually exclusive with KVM_MEM_READONLY").
