[v3,05/10] Rename MMUAccessType to MemoryAccessType

Rename MMUAccessType to MemoryAccessType and MMU_*
constants to corresponding MEM_* constants, so it would be possible to
re-use these defenitions in other, non MMU-related, contexts.
